CCAD requires each student to provide the information contained within this webform.
You will be guided through the various charges that may be billed to you and the credits that are available to reduce your costs. You will want to have Self Service open or the email that contains your Financial Aid Offer Letter. At the end of the form, you will be required to outline your plan to cover the costs for the semester. This is not a contract and you can change your plans but this helps us know how to help you.
Off-campus housing must be approved for first-time/freshman and sophomores.
Most first-time/freshman live in Schottenstein.
Sophomores get priority for living in DSA.
Provide the total amounts for each award type listed below. You will find these amounts on your Financial Aid Offer Letter or on Self Service.
The following questions will work through options for paying your out-of-pocket balance, including federal student loans that are available.
The CCAD Financial Aid Office will process federal student loans for you based on the information you provide in the next several questions.
If you are borrowing student loans for the first time, you must take additional steps:
Both of these steps must be done ASAP at www.studentaid.gov
On your offer letter or Self Service, you can see the maximum loan(s) available to you. How much of those loans do you wish to borrow? If there are two loans displayed — Subsidized and Unsubsidized — add the amounts together and enter the total here if you are borrowing the maximum.
Independent students can borrow more but check your Offer Letter or Self Service for your available loans.
If you request less than the total loan amount on your Offer Letter, we will always give you Subsidized Loans first, based on eligibility.
There is a small origination fee charged by the government so the total loan amount that will credit to your bill after that fee is subtracted will be adjusted.
$12,500.00 is the maximum amount.
PLEASE NOTE: These are estimated based on the information you have provided on this form. This could be incorrect or could change. Course-specific fees and the cost of a parking permit are NOT factored in here. This was calculated using the total charges minus your total aid, including the loans you indicated you will be borrowing.
Next, we are listing your payment options. Indicate how much of the out-of-pocket cost for the anticipated charges you will be paying using each option. Fill in as many options as is appropriate. As you enter amounts in the various payment options, you will be given additional guidance on how to secure each payment option. This is not a contract and you can change your plan at any time but this helps us work together on getting your balance paid.

Personal Funds: Make sure you check your email for billing statements with instructions on how to pay the balance each semester. You will be responsible to make your payments on time or will be at risk of late fees or being dropped from your courses if the fees are not paid.
Monthly Payment: Be aware that our monthly payment plan is interest-free but includes a $35 enrollment fee per semester. Please note that indicating your plan on this worksheet is NOT signing you up for a payment plan and you are not locked into this option. You will be given instructions from the Bursar's Office when the bills are distributed.
Parent PLUS:Please note: This form is not the application and parents MUST initiate their application at the federal site, www.studentaid.gov to apply for the PLUS loan. When your parents are completing their PLUS loan application, they must indicate the amount they choose to borrow. There is an origination fee charged to every federal parent loan so your parent will need to borrow a little more in order to meet the amount you indicated. Your parent may borrow additional money but you can see the maximum eligibility on your Offer Letter. If approved, your parent must have a valid Master Promissory Note on file with the government. If denied, your parent can either get a co-signer, they can appeal the denial, or you (the student) can borrow more loan money. Alternative/Private Loan: You may borrow more to help you pay for other costs not billed by CCAD but we encourage you to borrow wisely. You can compare alternative loan options available by visiting choice.fastproducts.org/FastChoice/home/303900. These are credit-based loans and if you do not have a sufficient credit history, you will need to include a co-borrower on this loan in order to be approved. You are responsible to go through the application process and get approved before your tuition bill is due. If you are late with this process, you will be at risk of being charged late fees and, ultimately, being dropped from your courses for non-payment. Alternative loans can take 30 days or more to process so be sure to give yourself enough time.
Veteran's Benefits: Our Registrar will work with you to utilize your Veteran's Benefits but you can also determine your eligibility and apply for your benefits at www.va.gov. Please note that VA benefits may not cover all of your costs at CCAD. It is important to have a plan to cover any remaining costs not covered by the VA.
